INSTRUCTIONS: Providing Leadership Feedback

Helping everyone become the best version of themselves is integral in building high performance teams.

Accurate, detailed, and timely feedback helps every employee grow personally and professionally.

There is no better way to gather this feedback than to solicit input from those with whom they work most closely.

Step 1:

Show your support by providing thoughtful feedback!

 

When you are asked to give feedback, you'll receive an invitation from [email protected].

Click on the link to be taken to the sign in page.

NOTE: If this is your first time logging in or you've forgotten your password, use the "Forgot Password" Link at the bottom left of the sign in page.

Once logged in, you will see a notifications link in the top right corner of your home page; clicking on the link will take you to your list of action items.

Step 2:

When you begin to give feedback, you'll be given instructions on the rating scale.

When ready, select the Provide Feedback button at the bottom.

Step 3:

Read each of the 25 descriptors of essential leadership / management knowledge, skills, and abilities.

Select a rating:

N/O  => Not Observed
1 - 2 => Rarely
3 - 4 => Occasionally
5 - 6 => Often

Comments are helpful.

Step 4:

If you don't have time to finish providing feedback on all the elements, use the Submit button at the bottom of the page to save your responses.

If you have not finished providing feedback for each of the 25 elements, your feedback will be saved and you can log back in later to finish up your feedback.

Step 4:

The final section of the Leadership Feedback form includes free text fields where team members can offer their suggestions regarding:

  • Start doing
  • Stop doing
  • Keep doing
  • Other comments
  • Potential for growth and promotion

Many have found this to be among the most helpful and surprising source of feedback.

Step 5:

When finished, select the Submit button.

A pop up will thank you and let you know your feedback has been recorded.
